Step by step
Follow these in order — skipping a step is the most common reason a complaint stalls.

- 2Use the board's official complaint form — most now accept online submissions.
- 3Identify the physician by full name and, where possible, license or NPI number.
- 4Describe the timeline chronologically with dates and attach records.
- 5Sign the medical records release the board requests — investigations stall without it.
