Step by step

  1. 1Ask for the written discharge summary and medication reconciliation before you leave.
  2. 2Confirm follow-up appointments, who schedules them, and by when.
  3. 3Get hands-on training for wound care, injections, or equipment before leaving.
  4. 4Ask what warning signs mean call the office, and what means go to the ER.
Before you move on

Write down who you spoke with, the date, and what they promised. A short dated note is the single most useful thing you can have if this issue has to be escalated later.
