Medication safety
Most preventable harm at home starts with an unreconciled medication list.
Step by step
- 1Carry one current list: drug, dose, frequency, reason, prescriber.
- 2At every new prescription, ask: what is it for, what are the side effects, what should I stop taking?
- 3Use one pharmacy where possible so interactions are screened in one place.
- 4Ask about look-alike/sound-alike names and confirm the label before the first dose.
Before you move on
Write down who you spoke with, the date, and what they promised. A short dated note is the single most useful thing you can have if this issue has to be escalated later.
