How to request your records
A written, specific request is answered faster than a phone call.
Timing that matters
HIPAA: response within 30 days, with one 30-day extension allowed if explained in writing.
Step by step
- 1Address the request to the Health Information Management (HIM) or Medical Records department, not your doctor.
- 2Identify yourself with full name, date of birth, and dates of service.
- 3List exactly what you want. “Complete record” is often interpreted as a short visit summary.
- 4State the delivery format: electronic copy to a portal, secure email, USB, or paper.
- 5Sign and date it, and keep a copy of what you sent and when.
Before you move on
Write down who you spoke with, the date, and what they promised. A short dated note is the single most useful thing you can have if this issue has to be escalated later.
